What to expect from first contact to a completed, NDIS-ready report.

A structured online form capturing your background, goals and daily functioning, completed at your own pace from home, with help from a support person if needed. One of the most clinically important parts of the process — the more fully it is completed, the richer the report.
Nisha selects the most relevant internationally validated tools based on your intake — WHODAS 2.0, COPM, PHQ-9, GAD-7, Katz ADL, Lawton iADL and others as appropriate.
An in-depth interview via secure video or phone, usually 60 minutes. We ask about daily routines, safety, functional variability, informal carer support, harder days, and what happens when support is not available. No travel, no clinic.
Nisha writes and signs off every report, integrating the interview, validated results and supporting documents into a comprehensive, planner-ready FCA in clear, NDIS-aware language, linked to s.24 and s.34 of the NDIS Act.
