What a Seeds OT FCA involves

A structured clinical process — six layers of evidence

Six layers of information, brought together by clinical reasoning. Not a quick chat, but a thorough assessment designed to understand real daily life.

1

Detailed questionnaires

Completed before the appointment, in your own time.

2

Document review

NDIS plans, reports, letters and school information.

3

Validated tools

WHODAS 2.0, COPM, PHQ-9, GAD-7, Katz ADL and others.

4

Clinical interview

Around 60 minutes via telehealth, focused on real daily function.

5

OT clinical reasoning

Patterns of function across the whole week, not one appointment.

6

Written report

Clear, NDIS-aware, personally signed off by Nisha.

A good FCA is not just what can be seen in one appointment. It is what is understood by looking carefully at patterns across daily routines, supports, fatigue, risk and harder days.

How it works

The assessment process, step by step

What to expect from first contact to a completed, NDIS-ready report.

1

Contact us — no referral needed

Fill in the contact form and we get back to you within one business day. We confirm whether we are the right fit before you commit to anything. If telehealth is not appropriate for your needs, we will say so clearly.

5–10 minutesNo obligation
2

Detailed intake questionnaire

A structured online form capturing your background, goals and daily functioning, completed at your own pace from home, with help from a support person if needed. One of the most clinically important parts of the process — the more fully it is completed, the richer the report.

At your own paceSupport welcome
3

Validated tools, selected for your situation

Nisha selects the most relevant internationally validated tools based on your intake — WHODAS 2.0, COPM, PHQ-9, GAD-7, Katz ADL, Lawton iADL and others as appropriate.

Tailored to your caseInternationally validated
4

Telehealth clinical interview

An in-depth interview via secure video or phone, usually 60 minutes. We ask about daily routines, safety, functional variability, informal carer support, harder days, and what happens when support is not available. No travel, no clinic.

Report writing and delivery

Nisha writes and signs off every report, integrating the interview, validated results and supporting documents into a comprehensive, planner-ready FCA in clear, NDIS-aware language, linked to s.24 and s.34 of the NDIS Act.

What your FCA report covers and includes

A Seeds OT FCA report is not a template. It is a clinically reasoned document that explains your functional reality — clearly enough for planners, reviewers, therapists and families to understand and act on.

Functional domains assessed

Self-care, domestic tasks, mobility, communication, cognition, emotional regulation, community access and safety — tailored to the individual.

Risk without support

What happens when support is not available — one of the most important and often missing sections of a report.

Informal carer burden

What family currently provide, whether it is sustainable, and what would happen if it reduced.

Validated assessment results

Scores from WHODAS 2.0, COPM, PHQ-9, GAD-7 and other tools relevant to the situation.

NDIS-linked recommendations

Support recommendations connected to specific functional evidence, aligned to s.24 and s.34 of the NDIS Act.

Outcome tracking plan

Specific, measurable goals that allow progress to be tracked at the next review.

Pricing

Transparent pricing, no surprises

Fees published upfront. No hidden costs. No referral required. Self-managed and plan-managed NDIS participants welcome.

About our pricing

Why our cost is lower than most providers

Seeds OT is an independent practice, owned and run by an occupational therapist. There are no corporate shareholders, no franchise fees, and no management layers to fund. Every dollar goes directly into clinical work — that is how we keep fees at the NDIS rate without compromising on quality or depth.

Frequently asked questions

Everything you need to know before you book

What is a Functional Capacity Assessment?+
A detailed occupational therapy assessment that examines how a person manages everyday life — what they can do safely, consistently and sustainably across home, community, work and school. It goes well beyond diagnosis to explain how disability affects real daily function, and is commonly used to support NDIS access requests, plan reviews and support planning.
Why isn't a diagnosis enough for the NDIS?+
Two people with the same diagnosis can have entirely different support needs. One may manage many daily tasks independently; another may need constant prompting or assistance just to get through a morning. The NDIS needs functional evidence — not just what condition a person has, but how it affects their daily life, participation, safety and the support they actually require.
Is a telehealth FCA as thorough as in person?+
For most referral questions, yes. A well-structured telehealth FCA uses validated tools, pre-assessment questionnaires, document review, clinical interview and OT reasoning. Many functional difficulties are not visible in a single clinic appointment — they show up in daily patterns across the week, which a structured telehealth process is designed to capture. Where in-person assessment is clinically necessary, we will say so.
Who writes my report?+
Nisha Bal writes and signs off every FCA, without exception. Interviews may be conducted by experienced OTs within the Seeds team, but Nisha selects the tools, completes the clinical reasoning, and writes the final report in every case. There is no junior staff producing reports without senior oversight.
How long does the whole process take?+
The telehealth interview is usually around 60 minutes. Reports are delivered within two weeks of receiving all information. Current wait from booking to delivery is typically 2–6 weeks, occasionally up to 8 weeks during busy periods, so we recommend booking as early as possible.
Do I need a referral to book?+
No referral is needed. Fill in the contact form — we prefer email because it lets us read your situation properly and give a more thoughtful reply before you commit. Self-managed and plan-managed NDIS participants are welcome to get in touch directly.
Can I claim through my NDIS plan?+
Yes — self-managed and plan-managed participants can claim the assessment cost. We bill at the standard NDIS OT rate of $193.99/hr. If you are agency-managed, contact us to discuss options.
What if I can't complete the online forms?+
We can email the forms as a document so you can type your answers at your own pace. If you cannot complete written forms at all, we can adapt the process with a longer interview instead. This may affect the cost, which we will discuss with you first. Just get in touch and we will find a way that works.
